The topic of this talk is perianal Crohn's disease, which is one of the most complex pathologies in inflammatory bowel disease. Management is very complex and is incapacitating for patients. 


Dr. Garcia-Duperly says that patients with this disease have a more than a 40% chance of having an anal fistula or some sort of fistula during their lifetime. Between 30% and 40% of these patients will undergo operations on the perineum because of these fistulas.


Moreover, there is a very high rate of recurrence in fistula extracts in patients with perianal Crohn's disease.  The surgeon can proceed with the clinical or intestinal manifestations in 30% of patients. 


Nevertheless, the most frequent association of perianal Crohn's disease is rectal disease, which makes treatment more difficult because a completely healed perineum and rectal mucosa are needed.
